A positive Volkman’s sign in a full blown case of Volkman’s ischemic contracture indicates

1.Constant length phenomenon of the flexor tendons
2.That gangrene of the fingers is imminent
3.That the forearm compartment requires a fasciotomy
4.That the fracture fragments are diplaced

In Volkman’s ischemia it is possible to extend the fingers fully only with the wrist in full flexion. On extending the wrist the fingers are flexed at the interphalangeal joints. This is because when the wrist is extended the flexors which are shortened and hence in constant length pull the fingers into flexion.
