Bacillary layer detachment (BLD)

👉Refers to a split in the inner segments of the photoreceptor layer between the myoid zone (MZ) and the ellipsoid zone (EZ).

👉A novel OCT finding that has been described in the acute stage of inflammatory or infiltrative lesions of the choroid which decrease the outer retinal perfusion (?surrogate for underlying choriocapillaris ischemia):

❗️Acute VKH syndrome (called “outer retinal cystic spaces”/ “septate neurosensory detachment” / “subretinal fluid with fibrin”)

❗️APMPPE

❗️Toxoplasmosis

❗️Posterior scleritis

👉Disappears with therapy, with rapid restoration of the outer retina and corresponding visual gain
