Can someone explain

And who is responsible in both

Last clear chance is suppose u have advised a patient for regular antiseptic dressing but the patient didn’t do and came back to you with infection. U advised the patient for regular dressing again but u urself didn’t do it the next time patient visited you and the patient lands up into septicemia or osteomyelitis etc.
