Valvular lesions:

  1. Fixed cardiac output lesions like aortic stenosis, mitral stenosis-
    Anesthesia of choice: As these patients cannot increase their cardiac output they will not be able to correct the hypotension seen with central neuraxial blocks( spinal/epidural) making GENERAL ANESTHESIA as anesthesia of choice.
    During GA, induction agent of choice will be Etomidate, maintenance to be done with Isoflurane and muscle relaxant of choice will be Vecuronium.
  2. Regurgitant lesions (AR/MR)
    As hypotension is beneficial in regurgitant lesions CNB may be preferred over GA.
    During GA anesthetic management will same as for fixed lesions.
