Continuation of anesthetic management of diseases:
CVS:
Valvular lesions:
- Fixed cardiac output lesions like aortic stenosis, mitral stenosis-
Anesthesia of choice: As these patients cannot increase their cardiac output they will not be able to correct the hypotension seen with central neuraxial blocks( spinal/epidural) making GENERAL ANESTHESIA as anesthesia of choice.
During GA, induction agent of choice will be Etomidate, maintenance to be done with Isoflurane and muscle relaxant of choice will be Vecuronium. - Regurgitant lesions (AR/MR)
As hypotension is beneficial in regurgitant lesions CNB may be preferred over GA.
During GA anesthetic management will same as for fixed lesions.
