HBV serology , continued. …

HBsAb if positive alone , indicates vaccination , and the tire should be checked and if inadequate , booster dose is required .

HBcAb IgM indicates recent infection or flare of chronic infection .

HBeAg (envelope ) is a marker of infectivity and if positive , indicates high HBV DNA levels and highly infectious patient .

HBeAb is a marker of seroconversion and indicates lower infectivity .

So…

HBsAg +HBcAb IgM positive = recent infection .

HBsAg +HBcAb positive = chronic infection

HBsAg + HBcAb +HBeAg positive = chronic highly infectious

HBsAg + HBcAb + HBeAb positive = chronic carrier (less infectivity ).

HBsAb only positive = prior vaccination ( check titre for adequate immunity )

HBsAb + HBcAb positive = prior infection , cleared it .

Note …In some mutant forms HBsAg and HBsAb can coexist and the effect of this on the course of the disease is still unknown …(forget it for now )
