in this as upgaze palsy(superior oblique) and lateral gaze palsy(lateral rectus ) involvement answer should be 6 th nerve palsy?

It’s CPEO. chronic progression , ptosis so 3rd nerve, lateral and upward movement restriction so 6th nerve.

does Diplopia occur in nerve palsies and in this Diplopia is not mentioned

not always… but in this case diplopia and restriction of movement have happened together .and also there is no other history . so it’s a nerve palsy…

Ptosis cannot occur in 6th cn palsy.
