Regarding Prostate Cancer Treatment:
Correct me if am wrong:
- Do nothing: In elderly > 75 years with serious co-morbidities with life expectancy of less than 10 years
(monitor, “watchful waiting”)
- Organ-confined disease.
(Early) • Radical Prostatectomy
- Locally advanced disease. (Organ involved + capsule invasion or seminal vesicles involvement)
Two months of Androgen deprivation therapy (ADT) to suppress the gland followed by External Beam Radiotherapy.
- Metastatic disease
• Androgen Deprivation Therapy (Palliative)
Bilateral orchidectomy – If spinal cord compression
Or Medical castration using luteinising hormone releasing hormone agonist like Goserelin.
- For Hormone refractory Prostate cancer:
Chemoradiation therapy
