This infarction is "transmural" because it extends through the full thickness of the ventricular wall

A 68 years old male with history of chest pain 3 days duration , No medical or drug history. He went to the ED and ECG has been done for him shows inverted T- wave and after 3 hours the pain started to improve then he was leaving the ED against medical advice(LAMA), after 24 hrs he passed away .
A proper autopsy performed in the IMLD and the cross section through the heart shows the larger left ventricular chamber and the small right ventricle. Extending from the anterior portion and into the septum is a large recent pale myocardial infarction. The center is tan ( coagulative necrosis)with surrounding zone of hyperemia and hemorrhage .
