Warfarin associated bleeding
Prothrombin concentrate( not cryoprecipitate) or fresh frozen plasma + IV vit K +stop warfarin only with life threatening or severe persistent bleeding or need for rapid reversal before urgent surgery .
IV vit K + stop warfarin if mild to moderate bleeding or high INR > 8 with bleeding risk ( IV vit K is preferred over oral ) .
Just stop warfarin if high INR (5-8) and no bleeding and recheck after 24 hours .
Resume warfarin after drop if INR below 5 and based on clinical judgement according to the condition .
