  1. Observership certificate

  1. Log book includes

a) Front page: your name, clinic name, clinic address, contact supervisor name, supervisor registration, date of supervison

b) Table (Date, patient initials, complaint, +v history, +v signs, investigations, management, NB and supervisor signature)

c) Detailed notes: details of the above table

NB: the above documents are my own design, feel free to adjust as you and your supervisor think

The log book and the certificates are counted a lot in your PESCI/ registration and also could be considered as CPD activities
