A 75-year-old male with angina has been increasingly moody and irritable for five weeks. He has lost interest in sex and his favorite leisure activities and nothing seems to cheer him up.

He has difficulty falling asleep at night and his appetite is decreased, although he has not lost any weight. His heart medications have not been changed for the past year.
