why wont we consider this person as a low risk case and consider suppuration > 15 mm when nothing as risk factor group has been mentioned?

Usually >10 mm is considered positive.

That criteria is published by CDC, intended for use in USA, which is a low prevalence country for tuberculosis.

India on the other hand has the highest disease burden of TB in the entire world. All indian patients are by default considered as high risk.
