Biopsy if in case of irregular cycles to rule out polyp and cancer

Ma’am you told in 4th edition that one of prerequisite of myomectomy is hysteroscopy+endometrial Biopsy if in case of irregular cycles to rule out polyp and cancer.My doubt is as if female comes with bleeding symptoms we do TVS first and there we get to know that tha thickness is below 12mm or above 12mm as we do myomectomy in females who want to concieve in future so these females are less then 45 yrs of age. So mam on tvs we get to know whether we have to go for Biopsy or not. Is it we do biopsy in every patient who have irregular cycles before so myomectomy or we do in those patients who have more than 12mm thickness on tvs

Endometrial biopsy : indications vary by age group and the presence of risk factors

  • post menopausal patient’s with any uterine bleeding and /or endometrial thickness >= 4mm

  • All patient’s > 45 yrs of age with frequent, heavy, and /or prolonged bleeding

  • patient’s < 45 yrs of age with frequent, heavy, and/or prolonged bleeding who are at high risk for endometrial cancer ( risk factors include obesity, polycystic ovary syndrome, Type 2 diabetes, Tamoxifen therapy, Lynch syndrome) and /or have failed medical management

Endometrial biopsy is done if females age is > 45 years irrespective of endometrial thickness.

In repr age it is done if she has a risk factor like pcos , lynch 2 , endometrial thickness > 12 mm
