Decrease the dose depends on what TSH or FT4?

in hypothyroidism whether we need to increase or decrease the dose depends on what TSH or FT4 ? And whether we need to increase or decrease the dose by 12.5 ?

Patient on TSH > 100 was started on thyronorm 100 microgram and now TSH is 25.45 should I reduce the thyronorm to 88 microgram ?

L-thyroxine is the mainstay of hypothyroidism treatment and is started at the dose of 1.7 ug/kg/day. Monitoring of the patient is based on TSH. Free T3 and free T4 do not have much value in monitoring. TSH values that are slightly out of range may be corrected by a single dose increment or decrement, such as increasing from 100 to 112 μg or decreasing from 175 to 150 μg, thus in multiples of 12.5ug. As the TSH in the patient is down-trending, it would be best to wait and monitor after 8 weeks of therapy at the same dose. Thank you.
