isn’t fractional curettage the gold standard in case of post menopausal bleed or any AUB.?
As Ques is mentioning : post menopausal AUB suspected to have AN ENDOMETRIAL LESION : so it’s likely a focal lesion . And on doing curretage WITHOUT Hysteroscopy ; a significant part of uterus can be left behind (as approx 60% of curretage specimen sample less than half of uterine cavity if done without Hysteroscopy as just curretage is a blind procedure ) . So better to go for hysteroscopy and take biopsy from suspected lesion site as you will be able to visualise entire cavity clearly . Also because main moto of doing fractional curretage or endometrial biopsy is to take sample for histopathological examination which anyway we will get . So i think this should be the reason . Otherwise best/gold standard is always Fractional curretage but WITH Hysteroscopy ( not just fractional curretage )
