Could I please get guidance regarding the following:

If we plan to do specialty training at ST3/4 level post MRCP?

Is it necessary to work in acute medicine/ general medicine in the NHS to get core competences signed off, or can we work locum shifts in medicine( as a second job), and get it signed by a consultant then?

If you mean core competencies in practical skills, then yes, even a locum shift sign off will count as long as the person signing off is a registrar /nurse /cons. It will have to be a summative dops to call it ‘sign off’.

I am not sure though how other requirements of a crest form can be signed off by locum shift cons, as:

a) you perhaps will not work with them for a period of 3 months minimum as locum shifts are not regular (unlike rota), and you need someone who has supervised you or worked wit you for a minimum for 3 months for a sign off.

b) if you’re on a visa, that limits the number of locum hours per week, so it might not give you enough time with a specific cons for them to be satisfied to sign off the crest.

Best way still would be your regular job.
