""7 days back at around 1 pm I got a call from one of a colleague Pediatricians stating that he has just seen a 10 yrs old boy with some stab wound over chest and boy is in shock so he has directed them to come to Artemis.

Understanding the gravity of situation we (Pediatric team) reached to ER to receive the boy. With in 5 minutes of us reaching there the boy arrived. As he was placed on ER bed, he had no respiratory efforts and bradycardia and soon went into asystole. CPR was initiated and suspecting it to be hemothorax/pneumothorax, emergency ICD placement on left side was done. Around 500 ml of frank blood came out with a gush and his cardiac activity reappeared only to disappear again so CPR continued. He was intubated in between and O- blood was transfused. Pediatric team along with Dr Rajesh from ER and Dr Nitin Goel, our pediatric surgeon along with nurses and GDA were on their toes to revive him. Recognising the need, Dr Aseem Srivastva, our pediatric cardio thoracic surgeon was called. As luck would have it he was on his way to put someone on ECMO at that time and he received the call just in that moment and he came rushing down to ER. A decision was made to move the child to OT to explore. After 40 minutes of CPR, he again got his cardiac rhythm back but his pupils were fixed and dilated. His initial venous blood gas showed a pH of 6.4 and lactate of 24. However we all decided that let’s give it a chance and he was wheeled in OT. After reaching OT he again went into asystole and CPR started. Dr Aseem decided to go ahead with procedure and chest was open. There was blood pouring out like anything and he found that there was a penetrating wound in rt ventricle which he repaired heroically. After surgery he was shifted to pediatric cardiac ICU for further care.

Today this boy has been discharged home and he moved out of hospital walking with absolutely intact neurology.

From ER to Blood bank to OT to pediatric and pediatric cardiac team along with Dr Aseem and his team worked in cohesion to make it possible.

In my whole clinical experience this is the first time I have seen a child with pH of 6.4 going home walking.

Miracles do happen… and this is probably the best that we have seen amidst this pandemic.
