The reason behind posting this picture and summary is to encourage our health worker who are continuously giving effort to save the covid-19 critically ill patients .
It all started with fever of 102 F and dry cough .He felt difficulty in breathing acutely even on rest . He was rushed to hospital with those complaints and admitted in ICU .The blood work and et-aspirate turned to be SARS-COV2 RT-PCR positive .Patient with bilateral viral pneumonia unable to maintain the oxygen saturation not even on high flow oxygen. Intubation needed and done promptly in birgunj hospital . On DOPR patient was transferred to GIH-ICU covid-19 in its worst, with the ventilator support with the help of GIH-EMS .
The diagnosis
1)Critically ill covid-19
2)Severe ARDS
3)Ventilator associated Pneumonia (Acinetobacter )
4)BPAD
In GIH-ICU the mainstay of treatment was
- Ventilation
Proning/ supine position of 3 session - Antibiotics
3)Dexamethasone
4)Remdesivir
5)plasma convalescent Therapy
6)Clexane
He was successfully extubated on the 8 th day .He was kept in GIH-ICU covid-19 for close vigilance as his COVID-19 RT-PCR was positive with bipolar affective disorder was persistence and until medication was optimized .
Finally today he was discharged from GIH-ICU to GIH-ward with COVID-19 RT-PCR negative and psychologically stabilize . Total stay in icu was 17 days .
