Age (especially if the patient is child) or weight of patient (if dose of medicine is calculated based on weight).
The superscription which is simply represented by the sign (Rx) its derived from an abbreviation for recipe,
The inscription this is general body of the prescription, it comprises a list of the ingredients and the quantity of each to be used or supplied
Name of medicine dosing frequency
Its dose form and strength. Duration (length) of treatment
The subscription direction to dispenser
Date prescription is written the prescriber’s name, signature and
(the address of the prescriber is usually printed at top of the prescription paper).
