First neonate discharged succesfully after mechanically ventilating for 5 days from province hospital surkhet at 14 DOL
. Referred on first day from dailekh(3 hour journey) to surkhet case of severe MAS/HIE grade II, came with active seizure, respiratory distress , started on cpap, failed CPAp ,kept in mechanical ventilation for 5 days, seizure controlled on two antiepileptic, developed septic ileus with abdominal distension, inproved on 7 day and finally of oxygen on day 12. Discharged on 2 antiepileptic. Though i am worried baby may have neurological sequelae…lets hope for positive outcome. At least 30 cylinders finished, another baby on same day succumbed due to unavailability of ventilator as we have only one neonatal ventilator. With oxygen plant not working, frequent power cut, life saving drugs unavailable… lack of resources, and inadequate monitoring… saving life is very very difficult in periphery… even a life is saved there is chance of sequelae due to delayed management.
