In a 58 yr old f , LH would be obviously elevated , why should it be an indication for surgery?

That would be physiologic elevation of LH. Whereas prolactin secreting pituitary tumor would respond to Surgery while other functional tumors of pituitary don’t respond to medical MX and need surgery.

If you mean a 58 year old menopausal woman, she will have physiological elevation of luteinizing hormone. However in case of an adenoma the luteinizing hormone level will be extremely high. More than the physiological limits even for postmenopausal woman. In this question only prolactin can be treated by medical management ( which is clearly mentioned to be the first line of management in case of a prolactinoma). Hence that is the answer
