A 36 yrs old female pt C/O recurrent episodes of diminution of vision in both eyes

A 36 yrs old female pt C/O recurrent episodes of diminution of vision in both eyes. She was treated with steroids and symptoms improved. O/E :. RE ;6/60 LE ;6/18 and colour vision is defective in both eyes. She also develops spastic paraplegia . Diagnosis ?

A. NMO

B. MS

Ans given as NMO but symptoms are going with optic neuritis with spastic paraplegia (UMN lesion) means CNS demyelination that is MS .

Why it’s not MS ?

How to differentiate B/w two ?

MS mostly presents with unilateral optic symptoms
