A 45 year old Diabetic lady operated for hip displacement and on urinary catheter

A 45 year old Diabetic lady operated for hip displacement and on urinary catheter was referred to the OPD post OP day 10 with complaints of sudden onset dysuria, increased frequency and urgency since 2 days with fever and back pain.
The patient had fresh complaints of abdominal pain with hematuria this morning.
On Examination the patient had mild raised temperature, concentrated appearance and suprapubic tenderness.
A urine sample was sent to the Microbiology laboratory with the attached history.
The lab gave a report of positive culture with Proteus mirabilis.
The patient was put on oral Levoflox.
USG KUB was done to rule out renal calculi.

Diagnosis: Cystitis
Isolated organism: Proteus mirabilis

Proteus is a pleomorphic gram negative bacelli which usually occurs as a saprophyte in the nature. It is also a common human commensal surviving on the moist areas of skin and in GIT.

Properties of Proteus mirabilis;

Gram negative coccobacelli
pleomorphic: bacillary and filamentous forms
swarming on culture
putrid/fishy/seminal odor
Indole: -ve
MR : +ve
VP : -ve
Urease: +ve
Citrate: variable
TSI : ALK/ACID with gas and H2S
Ornithine decarboxylase : -ve
