A complex hormonal response

Conditions like asthma, pneumothorax, pulmonary embolism can all trigger a state of shock due to impaired breathing or circulation.
Shock can activate a complex hormonal response where ADH release increases to conserve fluids and maintain blood pressure. Sometimes, even non-pulmonary factors like central nervous system infections or trauma can cause SIADH.
The severity of the underlying condition (asthma, pneumothorax, PE) can also influence the likelihood of SIADH. More severe cases are more likely to cause significant shock and hormonal imbalances.
Emphysema is a chronic obstructive pulmonary disease (COPD) characterized by damage to the alveoli in the lungs. While it can cause shortness of breath, it usually doesn’t trigger the same degree of shock or stress response as asthma, pneumothorax, or PE.
