Triad of swelling, discharging sinuses and presence of granules (in the discharge)

Eumycetoma: caused by fungal agents
Madurella mycetomatis (black granules)
Curvularia
Fusarium
Aspergillus nidulans (white granules)

Actinomycetoma: caused by bacterial agents
Nocardia
Streptomyces
Actinimadura

Botryomycosis: mycetoma like condition caused by Staphylococcus aureus.

Eumycetoma Vs Actinomycetoma;

Eumycetoma
Single well defined lesion.
Sinuses appear late and are few in number.
Discharge is serous.
Black or white grains.
Produce Osteosclerotic lesions.
Grains: > 2 μm, ( made of fungal hypahe)

Actinomycetoma
Multiple lesions with I’ll defined margins.
Sinuses appear early in the disease, many in number.
Discharge is purulent.
Grains are white or red.
Produces Osteolytic lesions.
size of grains is < 2 μm, contains filamentous bacteria.
