Pearls for MRCP part-I & II:
Atrial natriuretic peptide secreted mainly from myocytes of right atrium and ventricle in response to increased blood volume
B-type natriuretic peptide (BNP) is produced mainly by the left ventricular myocardium in response to strain.
Arterial and venous smooth muscle vasodilatation
BNP causes increased glomerular filtration rate (GFR) and inhibition of sodium reabsorption,
NICE currently recommends BNP as a helpful test to rule out a diagnosis of heart failure
NT-proBNP are indicated for the diagnosis of LV dysfunction and to assess prognosis and response to therapy in patients with heart failure
Natriuretic peptides are released from the atria in response to atrial dilatation and compensate to an extent for the sodium-conserving effect of aldosterone in heart failure.
Elevated troponin is observed in acute MI, pulmonary embolus, septic shock, pulmonary oedema, myocarditis & CKD.
