Why TIBC decreases in Anemia of chronic disease?
Hepcidin is the major player in anemia of chronic disease…It blocks ferroportin in the basolateral membrane of duodenocyte and hence iron cannot enter circulation.Hence serum iron is low…Serum ferritin is an acute phase reactant and hence increases during inflammation.Since Ferritin is increased,transferrin decreases,hence less binding with transferrin .This causes decrease in TIBC
